How Much Does NEMT Insurance Cost? Real Numbers for 2026
By Coach Willena McGee, RN-BSN · AAA-Accredited NEMT Training Provider
For one wheelchair-accessible van, commercial NEMT insurance typically runs about $1,200 to $1,800 a month, depending on your state, coverage limits, drivers and vehicle. Insurers usually want a down payment of three months' premium before coverage starts. Some want six months or a full year, and each vehicle you add can require another two to three months down. In my experience, North Carolina and Maryland have some of the best rates in the country. Budget for it before you buy the van, because insurance is one of the biggest numbers in your working capital.
Insurance is the line item that surprises new owners most. Plenty of people mention it. Almost nobody tells you the real number until you've already committed to a van.
What NEMT insurance really costs
For one wheelchair van, expect roughly $1,200 to $1,800 a month for commercial coverage. That range moves with your state, your coverage limits, the vehicle, your drivers' records, and whether your contracts require higher limits. In my experience, North Carolina and Maryland have some of the best rates nationwide, and other states run higher.
The down payment nobody warns you about
Most insurers won't start coverage on a monthly premium alone. They usually want three months' premium down. Some want six months, and some want the full year up front.

Three months down: about $3,600 to $5,400
Six months down: about $7,200 to $10,800
A full year up front: about $14,400 to $21,600
It doesn't stop at the first van. As you add vehicles, insurers typically ask for another down payment, often two to three months' premium for each one. Growth has an insurance cost, and it shows up before the new van earns a dollar.
Why NEMT insurance is so expensive
You're insuring commercial transportation of people who may use wheelchairs, have medical conditions, or need hands-on help in and out of the vehicle. Insurers price that exposure. Add the higher liability limits facilities and brokers often require, plus a new company with no claims history to bring the rate down, and the premium climbs.
Personal insurance gets you off the lot. Nothing more.
Here's the biggest warning I can give you. You can drive the van off the lot on personal auto insurance, but commercial insurance has to be on that vehicle before you transport your first client.
Once it's commercially insured, that van is for passenger transportation only. No school pickups. No quick grocery run. No personal errands. Any other use can violate your policy and leave you accountable for any accident or incident in that vehicle. Treat it like what it is: company equipment with one job.
What drives your quote
Your state and county
The vehicle: wheelchair van, minivan or sedan, and its value
Coverage limits, including any higher limits your contracts require
Driver records and experience, yours and every driver you add
Claims history, or the lack of one as a new company
The number of vehicles on the policy
What you need beyond auto coverage depends on your state and your contracts, such as general liability, or workers' compensation once you hire. Confirm requirements with your state and a licensed commercial insurance agent before you sign anything.
Where insurance fits in your startup budget
Insurance isn't a one-time cost. It's a monthly cost you carry while your trips ramp up, plus a down payment you need in cash on day one. That's why it belongs in your working capital, not as an afterthought. In my startup model, working capital is the biggest line, bigger than the van. See the full startup cost breakdown. There's also a right order to getting insured, and getting it wrong can cost you time and money.
